I accept that participation in any Performance Space program (including classes, camps, clinics, memberships, and online sessions) is at my own risk, and I release and hold harmless Performance Space LLC, its staff, and affiliates from any liability for injuries, damages, or losses. If the participant is under 18, I confirm that I am the parent/legal guardian and consent to their participation.

PARTICIPATION WAIVER & RELEASE

Introduction

Thank you for your interest and participation in the classes, programs, performance training, strength and agility sessions, camps, clinics, memberships, videos, and other content that Performance Space LLC provides both in person and online. All material is provided for educational, entertainment, and informational purposes only. We strongly recommend consulting your physician or healthcare provider before starting any new nutrition or exercise program, particularly if you are pregnant, nursing, prone to injuries, or have any special medical conditions.

If at any time you feel dizzy, faint, or experience physical discomfort, you should stop immediately and consult a healthcare professional. Participants under the age of 18 may only participate with the involvement and consent of their parent or legal guardian.

Scope of Waiver

This Waiver & Release applies to all activities offered by Performance Space LLC, including in-person training sessions, camps, clinics, performance training, strength and agility programs, online videos and content, and live classes or streaming sessions.

Assumption of Risk

I acknowledge that participation in these activities involves inherent risks, including strenuous physical activity, athletic movements, collisions, falls, contact with other participants, use of equipment, gym or facility hazards, balls, hoops, and training tools, as well as dehydration, overexertion, or other physical injuries. I voluntarily assume all risks associated with participation and understand that injuries can occur despite reasonable care.

Release of Liability

In consideration of being allowed to participate, I hereby release, waive, and discharge Performance Space LLC, its owners, officers, employees, agents, and representatives from any and all liability, including claims arising from ordinary negligence. This release covers injuries, damages, or losses resulting from participation in any activity, use of equipment or facilities, or instruction provided by staff. I understand that this waiver applies even if Performance Space LLC is partially at fault, except where prohibited by California law.

Medical Authorization

I authorize Performance Space LLC staff to provide basic first aid and to call emergency medical services if necessary. I understand and agree that I am responsible for any medical expenses that may result from participation.

Health and Fitness Certification

I certify that I, or my child, am physically fit and capable of participating safely in the activities provided. I confirm that there is no medical condition preventing participation and that I am responsible for informing Performance Space LLC of any changes to health status.

Behavior and Conduct

I agree to follow all rules, safety guidelines, and instructions provided by Performance Space LLC staff. I understand that staff may remove any participant whose behavior creates a risk to themselves or others.

Photo and Video Release

I grant permission for Performance Space LLC to capture and use photographs or videos of me, or my child, for marketing, social media, and promotional purposes. If I do not consent, I will notify Performance Space LLC in writing.

Acknowledgment of Understanding

By signing below, I acknowledge that I have read and fully understand this Waiver & Release. I voluntarily give up substantial legal rights and accept full responsibility for all risks associated with participation.

If the participant is a minor, I certify that I am the parent or legal guardian and provide my consent to this Waiver & Release on the participant’s behalf.
